Improving Communication Gaps: Building Care Connections in Person

Official Launch and Meet & Greet

Please join us on June 17 for a provider meet & greet with Allied Health clinicians and community health nurses, along with the launch of our new team-based care tools. 

These materials were developed with Burnaby family physicians and nurse practitioners in mind. The new toolkit is intended to make it easier to understand who does what, how to connect your patients with the right supports, and what workflows to follow when coordinating care for shared patients.  

Meet your Home Health team 
This event is an opportunity to meet the Home Health clinicians and community health nurses (CHN) connected to your practice and build relationships that can support smoother coordination of care as we go forward.  

The event will include a short presentation to introduce the new materials, followed by dedicated time for networking and conversation with Home Health, PCN, and family physicians.  

Dinner will be served. 

About the project 
Improving Communication Between Family Physicians, Community Health Nurses, and PCN Allied Health is a BDFP Shared Care project focused on strengthening communication and coordination among providers caring for shared patients in Burnaby. 

Building on last October’s team-based care mapping work, our virtual feedback session in May, and ongoing collaboration between Burnaby PCN and Home Health management, this event will launch the newly developed materials and create space for providers to connect across teams.
