Meet the staff members of the Burnaby Division of Family Practice.

James Musgrave
James Musgrave offers a unique mix of well-respected management and leadership skills developed within the context of community-based health and well-being programming and services.
James joined the Burnaby Division as Executive Director in January 2025, bringing with him a strong, collaborative, and strength-based leadership style, honed through work within government and the non-profit sector. A calm and stable presence, James aims to provide the Division with a strong foundation for growth, connection, and creativity in the Burnaby community.
James also has extensive work experience within communities and across all levels of government to develop cohesion and alignment to meet an identified need. He excels in working across systems and bringing them together to support a common community goal or vision.
James likes to interact collaboratively with participants, staff, volunteers and systems alike, both inside and outside the organization to develop capacity and capability within the individual, organization, and community as a whole.

Hinah Chaudhry
Hinah Chaudhry joins the Burnaby Primary Care Network as PCN Manager, Team-Based Care & Clinical Teams, bringing more than eight years of experience in nursing, clinical operations, and primary care leadership. A Registered Nurse with a Perinatal Specialty and a Master of Business Administration, Hinah has led multidisciplinary teams across two major health authorities in the Lower Mainland, as well as in private-sector clinical settings.
Most recently, she served as PCN Manager with the North Shore Division of Family Practice, where she worked in direct co-management with the VCH Clinical Operations Manager to align clinical operations, strengthen team-based care and attachment, and advance integration across community and health authority settings.
Hinah brings deep experience in optimizing workflows, leveraging data to drive quality improvement, and fostering strong, collaborative relationships across primary and community care. We’re thrilled to welcome Hinah to the Burnaby PCN team and look forward to her partnership and leadership in supporting our clinicians, teams, and program partners.

Matt Cooper
Matt Cooper is an experienced business and financial professional who brings years of hands-on management experience from the corporate world to the non-profit healthcare space. As Program Manager, he oversees the robust Long Term Care Initiative – comprised of 30 physicians and 12 care homes – organizing 24/7 physician coverage, and supporting best practices and quality improvement across Burnaby. Matt is passionate about equitable and accessible healthcare, and business efficiency! He enjoys engaging with physicians about their perspectives as owner/operators.
In his spare time, Matt enjoys powerwalks, swimming, reading nonfiction, and exploring culture through cooking. He also enjoys spending nice days at the beach with his senior Labrador, Benson.

Leona Cullen
Leona brings more than 35 years of administrative and operational expertise to her role as Operations Manager with the Burnaby Division of Family Practice. Having served as the Interim Executive Director until March 2025, Leona provided steady leadership through a period of transition and played a key role in onboarding the organization’s new Executive Director.
With a strong background in non-profit management, health care administration, and community engagement, Leona now focuses on strengthening internal operations, optimizing organizational processes, and supporting the team’s day-to-day functioning. She continues to be a vital part of the Division’s leadership team, ensuring alignment between strategy and execution.
Leona has been with the Division since 2013, contributing to its growth and stability through her commitment to collaborative leadership and operational excellence.
In her free time, she enjoys music, movies, volunteering, and impromptu adventures with friends and family.

Elena Domasheva
Elena joined the Burnaby Division in August 2023 as our new Patient Attachment Coordinator.
She brings years of international experience in Human Resources and administration in the healthcare sector, as well as local experience in administration, customer service, and data entry. She has interest and passion to be part of the Canadian public healthcare system.
Elena holds a diploma in Healthcare Office Administration and has completed an MOA practicum.
In her free time, Elena enjoys spending time with her family and friends, getting out into nature and exploring beautiful places near Vancouver. She also enjoys travelling, reading, and practicing yoga.

Rissienne Gammad-Tiongson
Rissienne (Riss for short) arrived in Canada in February 2024 and joined the division as the Executive Assistant in March 2024. She brings valuable experience from her previous roles in government agencies, including the National Commission on Indigenous Peoples (Ancestral Domains Office), the Department of Justice, and the Philippine Postal Corporation. Her background also includes insights gained from her time in law firms before joining BDFP. Armed with a Juris Doctor degree and a B.S. in Legal Management, she provides a solid foundation to the team.
In her spare time, Riss indulges in humor by pretending to be a YouTube mom with endless resources and an unrealistic figure. However, her real joy comes from spending time with her family, especially her beloved toddler, Rylo. She believes that life is too short not to laugh every day, which contributes to her positive demeanor.

Clarke Gonzalez
Clarke joined the Burnaby Division of Family Practice in late 2025. She brings over 17 years of Office Administration experience across the insurance, education, and healthcare sectors, although her passion is the healthcare sector. With a BA in Communication as well as an MOA certificate, she values building strong connections and strives to bring out the human experience in every interaction.
Passionate about healthcare, her approach to work is with professionalism, compassion, and a positive outlook. Having a personality that is fun and energetic, Clarke enjoys creating an uplifting environment for those around her and connecting with people.
Outside of work, Clarke loves being outdoors and at the beach; walking, hiking, running, and finding new adventures with her dog, partner, and friends. She prefers summer over winter so has embraced the ever-popular paddleboarding and pickleball activities when the days are longer in her downtime. As a mom of two teenagers, life is always active and full of joy…as well as expensive!

Georgiana Ioica
Georgiana joined the Burnaby Division in February 2020. She brings previous experience in private and public healthcare organizations supporting Family Physicians and their practice teams, managing and overseeing day-to-day operations of various departments. She has a strong interest in Quality Improvement, clinical operations, health policy and is passionate about inspiring and driving change and translating complex processes into clear, actionable steps. Outside of work, you can find Georgiana enjoying nature, reading, traveling and spending time with her family and friends.

Veronica de Jong
Veronica draws on several years of interdisciplinary inquiry, first in academic research, and more recently within health service delivery. As a Canadian Institutes of Health Research (CIHR) funded Masters student at McGill University, Veronica investigated the psychological effects surrounding pharmacological prescribing and the doctor-patient relationship. Through her epidemiological and clinical trial work with Simon Fraser University’s Community Health Research Team, she had the privilege of listening to the experiences of many people dealing with chronic disease, or more broadly, their lives. Through an intersectional lens, she has lead implementation and evaluation work in secondary prevention telehealth programming with Fraser Health, among a wide array of public, private, and community partnership projects. Having joined the Burnaby Division of Family Practice in the throes of COVID-19, Veronica is thrilled to bring her insights into the broad reach of primary care. Happiest in the kitchen in the company of others, her wish is to promote wellness and build community through endeavours that nourish what we all share as humans.

Robbie Lacuna
Robbie joined the Burnaby Division in August 2020 initially as a MOA/Site Navigator. After a few months, he took on the role as Site Coordinator for the Burnaby Central Park Covid-19 Testing Site.
In May 2021, he became the Recruitment & Retention and Data Management Coordinator for the Division. Robbie brings with him years of experience in customer relations, business development and data management. He has a strong interest in public health and passion for community service. Robbie aims to help and improve healthcare in the Burnaby community.
In his free time, Robbie enjoys snowboarding, discovering new food and traveling the world with friends and family.

Huyanne Le
Huyanne joined the Burnaby Division in June 2022, Huey (preferred) is an experienced community programmer in both municipal government and not for profit settings. With a B.A. in Political Science and a Dialogue and Civic Engagement Certification both from Simon Fraser University, Huey has been working in DTES community services and local asset development in various capacities since 2009.
She currently holds the role of PCN Senior Manager, Community Partnerships and Engagement, supporting PCN governance work, Regional and Local Leadership Tables, and community building. Bringing an intersection of experience from Food Security Programming, Mental Health outreach, Family and Seniors engagement, Wellness and Recreation advocacy, and project facilitator roles; she is committed to a holistic and systems approach to community care.
Huey’s out of work passions are reading a book a week and exploring BC nature trails with her two dogs.

Laura Liu
Laura has a background in Sociology, Anthropology, and Biology and joined the Division in February 2025 as PCN Administrative Coordinator. She has many years of experience working as a Medical Office Manager in a busy family practice clinic. She brings with her a strong knowledge base for clinical project management, patient education, and client service. She has a passion for creating safe spaces to improve patient health and mental well-being.
Recreationally, Laura enjoys hiking, exploring new recipes, reading, and spending time with friends and family. She has a love for music and is always on the hunt for new songs to add to her workout playlist.

Tina Lý
Tina Lý is a storyteller and communicator, bringing years of professional communications experience in healthcare and higher education. With an expertise in community building and engagement, Tina is passionate about centering equity, inclusivity, and accessibility in her work.
Tina joined the Burnaby Division in August 2024 as a Communications Officer. In her role, she will support the Communications Manager in building the Burnaby Division’s brand, web presence, and communications strategy.
In her free time, Tina tries her best to be a little creative, whether it be through photography, videography, cross-stitching, baking, or zine-making. But more often than not, you’re likely to find her exploring coffee shops across the Lower Mainland in pursuit of the best pastry!

Roberta O’Brien
Roberta is an experienced community developer and project manager working in the social sector for almost two decades supporting health and well-being in various communities throughout the Lower Mainland and Fraser Valley. She has worked within community for several NPOs, ministries and academia throughout the Lower Mainland and Fraser Valley as an engagement specialist, researcher, evaluator, grant writer, media consultant, event planner, and mentor.
She has a BA in Geography and a diploma in computer science, both from University of the Fraser Valley. Her experience across sectors includes supporting health and wellness initiatives related to early and middle childhood brain research and development, food security, childcare, immigrants and refugees, vulnerable youth, climate and environment, violence against women, and much more.
She has a passion for social enterprises and is an avid climate and meteorology buff. In her free time, she enjoys travelling and exploring the local trails, rivers, and lakes with her dog.

Ahmad Masoud Rahmani
Masoud joined the Burnaby Division in June 2024 as both a Patient Attachment Coordinator and a Community Program Coordinator with our PCN. He is an internationally trained Medical Graduate (Canadian / Afghan) with over 15 years of experience in primary health care, having graduated from Kabul Medical University. Masoud also holds an MBA from India – Thailand and an MHA from the University of Minnesota in the USA.
Before coming to Canada, Masoud led Blood Safety Services for Afghanistan and consulted for the Canadian Embassy in Kabul. He has collaborated with several prestigious institutions, including Harvard Medical School, and has spent most of his career in primary health care, both internationally and in Canada. He has also been involved in many community engagement programs, including MOSAIC, where he coordinated with the PCN team and conducted various training programs.
Masoud has worked as a Research Assistant with the University of British Columbia and now serves on the Board of Directors of Umbrella Coop, promoting voluntary blood donation and being a regular blood donor himself. In recognition of his work, he received the Leadership and Management Award from USAID HQ in 2010.
Currently residing in BC, Masoud continues to promote medical education overseas through platforms like Can-Health (Cyberpatient and Lecturio). He has several internationally published papers to his name.
In his free time, Masoud enjoys nature, going to the gym, and playing soccer.

Maninder (Manny) Sahota
Manny joined the Burnaby Division in October 2017 as the Project Administrator. She worked at the BC Patient & Quality Council from 2011 to 2017 as the Program Assistant. She brings more than 15 years’ experience in office administration and customer service. She also served as an executive assistant for the Thames Valley Chamber of Commerce in the UK. Her office administration skills are combined with experience in employee training. Manny holds a Business Administration Diploma from CDI College.
Manny is a mom to two boys and in her spare time loves traveling and being with her family.

Melissa Sharp
Melissa Sharp is a communications professional who is passionate about advancing the health and wellbeing of her community. She brings years of experience as a communicator in non-profit and post-secondary spaces, with a focus on community engagement, public programming, and partnerships.
She began consulting for the Burnaby Division of Family practice in late 2020 to support pandemic response and brand development for the Burnaby Primary Care Networks (PCN). She joined the team officially in June 2022 as PCN Communications Officer and now leads communications strategy and implementation across the PCN and the Division as Communications Manager.
Outside of work, Melissa enjoys knitting, puttering in her balcony garden, and cycling around her neighbourhood.

Kamaljot (Jot) Sohi
Kamaljot (Jot) Sohi joined the Burnaby Division as a Shared Care Project Coordinator in December 2024. She is a professional with over five years of experience in healthcare operations, project coordination, and community engagement. Her career highlights include working as a Program Advisor with PHSA, where she supported emergency management operations across healthcare settings and providing administrative support during emergency responses.
Her academic background includes a BSc in Health Sciences from SFU and a project management certification, which underpin her strengths in project management and data-driven decision-making.
Beyond her professional roles, she is passionate about traveling, discovering new cultures, and spending quality time with family and friends. Additionally, she enjoys connecting with nature through outdoor activities like hiking and snowboarding.
Our Board
The Burnaby Division of Family Practice Board is currently comprised of five family doctors and two community representatives.

Dr. Leo Dai
Dr. Leo Dai, the current Chair of the Burnaby Division and a Board member for the past four years, has led key initiatives that have strengthened the organization and improved healthcare delivery in the community. His work has encompassed governance improvements, the development of Primary Care Networks (PCNs), Patient Medical Home (PMH) initiatives, interdivisional collaborations, and restructuring the organization to better serve its members. Throughout this journey, Dr. Dai has collaborated closely with partners such as the Ministry of Health (MoH), Doctors of BC (DoBC), Fraser Health Authority (FHA), and numerous community agency partners to ensure that initiatives align with the needs of the community and healthcare providers.
In addition to his role on the Board, Dr. Dai continues to practice as a longitudinal family physician, delivering patient care while managing a busy group practice. As a clinic owner, he understands the challenges and responsibilities of running a medical practice. He is deeply committed to integrating technology into healthcare while recognizing the importance of preserving the human element in medicine. Additionally, Dr. Dai is dedicated to making Burnaby an attractive destination for both locums and permanent physicians, recognizing this as vital to the sustainability of local practices.
Dr. Leo Dai is committed to ensuring that the Burnaby Division continues to meet the day-to-day needs of its members, providing the reliable support and resources that have always been a hallmark of the organization. He respectfully asks for your support in the upcoming election to continue building on the progress made and to work towards a stronger, healthier future for Burnaby.

Dr. Forson Chan
Dr. Forson Chan (MD, CCFP) is the current Vice-Chair of the Burnaby Division and Board Member. He has been practicing in Burnaby since 2020 and founded Oakmount Medical in 2023. He is also a past District Delegate for Burnaby with BC Family Doctors, a locum at the Metrotown Urgent and Primary Care Centre, and clinical instructor as part of the UBC Faculty of Medicine. Dr Chan also provides consults for patients with substance use disorders and ADHD.
Themes of Dr Chan’s work over the past 3 years include: opposing the corporatization of medicine; tearing down technical and administrative barriers that get in the way of good medical care; and improving how doctors and patients can navigate our complex medical systems. Recent involvements have included: leading a grassroots bid for PHSA’s Episodic Virtual Care contacts, Burnaby’s shared-care project on substance use, projects on Artificial Intelligence and AI scribes with Doctors of BC, and the centralized referral initiative from PHSA.
Dr. Forson Chan’s career reflects a commitment to patient care, finding solutions to problems in the current medical system, and addressing community health challenges. He believes in empowering family physicians to do what they do best: providing excellent patient care.

Justin Boehringer
Justin Boehringer joined the Burnaby Division of Family Practice Board in September 2025. He is a proud member of the Skeetchestn Band of the Secwépemc Nation through his father’s side, with white settler ancestry from his mother’s side.
As Associate Director of Indigenous Perspectives at the new Simon Fraser University School of Medicine, Justin’s work focuses on advancing Indigenous rights, self-determination, and systemic change in medical education. He is committed to embedding Indigenous ways of knowing across curriculum, admissions, and governance, and to ensuring education serves historically marginalized and under-served communities.
With the SFU School of Medicine focused on training future physicians in community-based primary care, Justin sees opportunities to strengthen connections between the school and local health systems. On the BDFP Board, he brings a collaborative spirit and a commitment to creating culturally safe, equity-driven, community-led health care solutions rooted in respect, reciprocity, and justice.

Simone Brandl
Simone Brandl joined the Burnaby Division of Family Practice Board in September 2025. With over 20 years of experience working in community, Simone brings a deep understanding of the diverse needs of Burnaby’s population and a strong commitment to advancing health and wellness.
As Executive Director of The Long Table, Simone has focused on increasing food security and reducing social isolation. During the 2020 pandemic, she developed a food systems program that led to the creation of nine community food hubs, which continue to support more than 12,000 people each week. This initiative helped inspire the City of Burnaby’s Food System Strategy, approved in 2024. For her leadership in this work, Simone was recognized as Burnaby’s Citizen of the Year.
Simone has also served on numerous community associations and committees in roles such as Secretary, Treasurer, and Board member. With a systems-oriented approach and values-driven leadership, she is dedicated to ensuring that initiatives address both immediate needs and long-term health outcomes for the community.

Dr. Vivian Liu
Dr. Vivian Liu is honoured to be a Director and Treasurer on the Burnaby Division Board of Directors. Having experienced the support of the Burnaby Division while transitioning into practice after completing her Family Medicine Residency, she felt welcomed and connected to her colleagues, patients, and services in her hometown. Dr. Liu is now eager to give back and support new-to-practice Family Physicians and other medical professionals.
As a Clinical Instructor at the UBC Department of Family Practice, the upcoming SFU School of Medicine, and a member of the Burnaby Division CME Committee, Dr. Liu is passionate about continuing medical education and fostering strong relationships with local specialist colleagues. Through her work at the Edmonds UPCC and as a locum throughout Burnaby, she has gained an in-depth understanding of the unique needs of each Burnaby neighbourhood and recognizes the barriers patients, especially unattached ones, face when accessing care. These insights have helped her highlight these needs as the Burnaby District Delegate on the BC Family Doctors Board of Directors.
Dr. Liu has served on the Primary Medical Home for Priority Populations Task Group, where she contributed to designing workflows for the new clinic at the Edmonds Centre for Healthy Communities. This clinic serves as the patient medical home for newcomers/refugees, expectant/new parents and their babies, people with complex mental health needs, and homeless/underhoused residents. In addition, she has collaborated on Shared Care Projects and is currently the Family Physician Lead on the Shared Care Steering Committee for the Burnaby Screening for Unattached Patients Project, which aims to improve access to screening for metabolic conditions in these patients.

Dr. Jennifer Moore
Dr. Jennifer Moore, a dedicated and experienced practitioner, has been a stalwart figure in Burnaby’s healthcare landscape since 1994. Specializing in longitudinal family medicine, with a particular focus on obstetrics, Dr. Moore has witnessed the evolution of healthcare in Burnaby firsthand. A pivotal member of the Board of the Burnaby Division from its early stages, Dr. Moore played a key role in shaping the organization and has continued to contribute actively even after completing their term in 2017.
Dr. Moore is currently involved in various committees, including the Hastings Brentwood LLT, the PMH, and the PCN steering committees. This commitment reflects her ongoing dedication to the healthcare initiatives in Burnaby. Furthermore, she has been a driving force behind a Shared Care project on mental health, demonstrating a profound commitment to healthcare solutions.
Having attended the Leadership and Management Development Program through the FPSC during her previous tenure on the Board, she brings a wealth of knowledge and a strategic mindset to her role. This unique combination of formal training and practical experience positions her as an ideal candidate for continued service.
Beyond the professional realm, she is deeply passionate about her vocation and the Burnaby community. Her interactions with community players and health authority figures have enriched her understanding and strengthened her vision for the well-being of Burnaby residents.

Dr. Tracy Monk
Tracy Monk brings a wealth of experience and passion to the Burnaby Division of Family Practice, where she eagerly anticipates the opportunity to serve her colleagues and community. Having observed the innovative and forward-thinking initiatives of the Burnaby Division over the years, she is excited to contribute to this impactful work.
Tracy’s extensive leadership experience includes:
– Served six (6) years on as a representative on the Family Practice Services Committee (FPSC)
– Former President of the Society of General Practitioners (SGP, now known as BCFD) and many years of service on the SGP board
– Physician Lead of Pathways BC, a collaborative non-profit project initiated by Divisions & FPSC
– Inaugural Chair of the Fraser Northwest Division
– Former Head of the RCH Department of Family Practice
– Previous board member and current clinical data stewardship committee member of the Health Data Coalition
Tracy’s leadership roles have provided her with valuable insights and skills that she is excited to bring to the Burnaby board. Her office is located in the new Lougheed PCN, initially part of the Fraser Northwest Division. With significant changes planned for the area, she sees an exciting opportunity to shape the future of medical care in Burnaby and to align efforts with the broader Burnaby PCNs. Tracy has a deep appreciation for the Burnaby Division’s engagement with community partners.
Tracy Monk is grateful for the opportunity to have served on the Board over the past year and remains eager to contribute to the continued success of the Burnaby Division of Family Practice.

Michel Pouliot
Michel is a professional and skilled Executive Director with a wide array of skills and experiences in leading and managing organizations. He worked as the executive director of Burnaby Family Life for 7 years and that is where he connected with the work of the Division of Family Practice as he represented community agencies at the Primary Care Network Steering Committee.
Michel is a passionate advocate for the needs of the most vulnerable members in our community with a strong track record of developing community partnerships. He is the Executive Director of Canadian Parents for French and focused on supporting French immersion and core French programs across the province and the Yukon.
