What is the Primary Care Network?
A Primary Care Network (PCN) is a clinical network of local Primary Care Providers (PCP) located in a geographical area, with patient medical homes (PMHs) as the foundation.
The Burnaby PCN is enabled by a partnership between the Burnaby Division of Family Practice, Fraser Health, and community service providers. Our PCN is based on a co-developed, comprehensive, and sustainable vision for the future for PCNs to enable access to quality primary health care that effectively meets the needs of Burnaby residents and patients.
The Burnaby PCN strengthens team-based care, increasing access and attachment for Burnaby patients. Our PCN is guided by the ten core attributes of Primary Care Networks in BC:

Burnaby has four PCNs based on municipally-defined neighbourhoods within the Burnaby area:
- Hastings-Brentwood
- Edmonds
- Metrotown
- Lougheed
What services does the network provide?
The Burnaby PCN encompasses a diverse range of health care services and community-based programs, including but not limited to:
- Two Urgent and Primary Care Centres
- Nine PCN Allied Health Services to support patients attached to Burnaby primary care providers
- The MOSAIC Community Clinic, providing longitudinal care for priority populations as well as postpartum care for unattached patients and their babies
- Various community outreach and health literacy initiatives, integrating medical and social supports in Burnaby
