DESCRIPTION:Join Dr. Vernon Ho Yuen\, Ophthalmologist\, for an upcoming CME sessions on cataract management in primary care. \n\n\n\nParticipants will earn 1.5 Mainpro+ credits from this session. \n\n\n\nThe deadline to register is Friday\, May 15\, 2026. \n\n\n\nContact us to registerPlease contact pmh@burnabydivision.ca to register for this CME session. \n\n\n\nAbout Dr. Vernon Ho Yuen \n\n\n\nVernon was born and grew up near Toronto. He completed medical school at the University of Toronto\, residency training in Ophthalmology and a Masters of Health Sciences at UBC and 2 years of fellowship training in Oculoplastic Surgery at the University of California Los Angeles and the University of Ottawa. He worked as a subspecialist in Oculoplastic Surgery in Halifax\, Nova Scotia\, then as a rural Ophthalmologist in Cranbrook BC.  \n\n\n\nIn 2020\, Vernon moved to Burnaby\, where he now works with the Ophthalmic Surgeons group at their offices in North Burnaby and at Metrotown\, and does surgery at Burnaby Hospital. Vernon loves living and working in Burnaby and is delighted to have an opportunity to present to the Burnaby Division of Family Practice. \n\n\n\nBDFP CME offerings

DESCRIPTION:A feedback session for Burnaby physicians\, nurse practitioners\, and Allied Health (PCN and Home Health) staff on our new set of tools to support team-based care. \n\n\n\nWe need your feedback to help improve our team-based care resources! \n\n\n\nBurnaby health care providers are  invited to a virtual feedback session on new draft materials developed to improve communications between family physicians (FPs) and allied health staff in the Burnaby Primary Care Network and Home Health. \n\n\n\nAbout the project:Improving Communication Between Family Physicians\, Community Health Nurses\, and PCN Allied Health is a BDFP Shared Care project that aims to bridge the gap between health care providers and how they provide care to their shared patients in Burnaby. \n\n\n\nBuilding on the team-based care mapping work last October\, and through ongoing work in collaboration with PCN and Home Health management teams\, we have developed a set of draft role-clarity and communication tools. \n\n\n\nWe now need your clinical expertise to ensure these tools will work in practice. \n\n\n\nWe’re asking for you to: \n\n\n\n\nReview the draft tools and materials informed by the team-based mapping work\n\n\n\nShare your feedback and advice to ensure these materials fit your current clinical practice\n\n\n\nIdentify areas of improvement before we roll out the final materials\n\n\n\n\nWhat to expect: \n\n\n\n\nBrief overview of project context and goals\n\n\n\nWalkthrough of the draft materials (e.g.\, role clarity\, workflows\, communication tools)\n\n\n\nGuided discussion and open feedback\n\n\n\nOverview of next steps and timelines\n\n\n\n\nParticipants will receive a DoorDash gift cardAs a token of appreciation for your time and efforts\, all Allied Health staff will receive a DoorDash gift card. \n\n\n\nFPs and NPs will also receive a sessional payment for their attendance. \n\n\n\nThank you so much for your consideration! \n\n\n\nRegisterPlease contact sharedcare@burnabydivision.ca to register for this webinar session. \n\n\n\nLearn about Shared Care
